Chaos starts reviving the villains from a panoply of mahou-shoujo series and sending them on continuity-crossing attacks. Nanoha's magitech has to fend off Viluy's nanobots, Joker shows off more reality-bending power than post-Rebellion Homura, and kidnapped Precure are imprisoned at the Galaxy Cauldron.
A mysterious rescue party works to gather the remaining heroines at a safe cross-dimensional hideout. Their ultimate goal is to protect and awaken Kaname Madoka, Tsukino Usagi, and Kinomoto Sakura -- the Three magical girls whose ultimate forms transcend all others in the multiverse.
(IIRC there isn't an official "final form" outfit for Sakura to wear...so I put her in the one that coordinates best with Madokami and Sailor Cosmos.)
